Symptomatic treatment in peripheral neuropathies

Treatment of peripheral neuropathies is guided by two general principles – treatment of the underlying disease and treatment to relieve specific symptoms regardless of etiology (symptomatic treatment). The latter depends on the type of the affected peripheral nerve fibers – motor, sensory and/or autonomic. The possibilities for improving the motor symptoms are limited. There is no effective treatment for muscle weakness.
Improvement in symptoms such as cramps and fasciculations is possible. Successful medication therapy is available for autonomous symptoms such as orthostatic hypotension, gastrointestinal, urinary, sexual dysfunction and abnormal sweating.
The most successful is the management of the neuropathic
pain, which is the major problem affecting the sensory fibers of peripheral nerves. Drug treatment of neuropathic pain includes several classes of oral and topical medications. Antidepressants and antiepileptic drugs are the first choice. Despite all the advances, the symptomatic treatment for peripheral neuropathies is unsatisfactory and needs future development.
